Following these steps will help make sure that your PCHS health center visit goes as smoothly as possible:

Phone any of our 13 health centers to schedule an appointment.

Please leave time to register by arriving at the Health Center 15 minutes before your scheduled appointment, or a half-hour in advance if you are applying for the Sliding Fee Program.

Before your visit, please make sure you have:

• photo identification
• your insurance card, if you have insurance
• your insurance copayment, or a minimum $10.00 payment if you are applying for the sliding fee program (for dental appointments, the minimum payment is $25.00.  Some exceptions apply, so please talk to your dentist)
• if you are applying for the Sliding Fee Program, you will also need to bring a copy of all household income for the past 30 days

At the Health Center:

• Sign in at the reception desk and have a seat.
• When your name is called, you will register and receive an Encounter form. 
• This form is your treatment record.  You will take it with you wherever you go during your visit -- first to Medical Records, then to your department, and finally to the cashier.
• At your department, sign in and the doctor or nurse will call you for treatment.
• If you have prescriptions that you would like to have filled at the PCHS pharmacy, be sure to drop them off.  Make sure to ask your doctor to submit the prescription for delivery if you are at a PCHS Health Center other than Alma Illery.
